Note a couple of things:
 

• Once you complete this form, you will receive an invoice from me within 48 business hours. Your order will be confirmed once you have submitted payment!
   

• If you would like to ask for a different day pick-up or have any other special requests, please include them in the box at the end of this form and I will let you know what I can do!
   

• Orders are due by Thursday, February 8, and will be ready for pick-up on February 13 or 14.
